College Admissions

A. High School Graduates

1. High School Report Card (Form 138)
2. Certificate of good moral character
3. Photocopy of birth certificate
4. Permanent Record (Form 137)
5. 2 copies of 2×2 ID picture

B. Transferees

1. Official Transcript of Records or Certification of Grades to be evaluated by the registration adviser
2. Honorable Dismissal
3. Photocopy of birth certificate
4. 2 copies of 2×2 ID picture

C. Married Students

1. Certified copy of authenticated Marriage Certificate or photocopy from the Philippine Statistics Authority (PSA)
2. Requirements listed under A or B
3. Pregnant student will not be allowed to enroll on the current school year. (If pregnancy occurs during a particular semester the student will be allowed to finish the semester in the other allied health programs, but will not be allowed during their clinical internship.)

D. Foreign Students

1. Permit to study and placement from CHED
2. Photocopy of the Alien Certification of Registration (ACR)
3. Photocopy of Birth Certificate
4. 2 copies of 2×2 ID picture

E. Certificate Courses

1. Official Transcript of Records or certified true copy of grades
2. Photocopy of Marriage Contract (for married female enrollees)
3. 2 copies of 2×2 ID picture

Enrollment Procedure

For Incoming First Year College

STEP 1: Proceed in the Student Affairs Office to take the Assessment. If passed, the student will be prepared for the enrollment.

STEP 2: Proceed to the Registrar’s Office and submit the enrollment requirements and fill out the Enrollment Form.

STEP 3:  Proceed to the Accounting Office for the payment process.

Go to the Cashier section for the tuition fee payment

Go to the Assessment section for printing of official registration.

 

For Transferees

STEP 1: Proceed to the Respective Departments for the assessment of credentials.

STEP 2: Proceed in the Student Affairs Office to take the Assessment Exam. If the student passed the exam and is ready for enrollment, he/she is required to pay PHP500 SDF.

STEP 2: Submit the enrollment requirements to the Registrar Office and fill out the Enrollment Form.

STEP 3:  Proceed to the Accounting Office for the payment process.

Go to the Cashier section for the tuition fee payment

Go to the Assessment section for printing of official registration.

 

For Current Students

STEP 1: Secure Clearance from the Accounting Office.

STEP 2: Proceed to Respective Departments, present the Clearance for Evaluation and Advising Subjects to be enrolled.

STEP 3: Proceed to Registrar Office for Printing of Assessment Form.

STEP 4: Proceed to the Accounting Office for the payment process.

Go to the Cashier section for the tuition fee payment

Go to the Assessment section for printing of official registration.

STEP 6: Proceed to the ICT Office for ID Validation.

STEP 7: Proceed to the College Library for Library Card Issuance/Validation.
